Thresholds, Illinois’ oldest and largest provider of mental health services, is seeking a clinician for Emerge West, our outreach service team for Emerging Adults which serves the Western suburbs of Chicago. Modeled after our successful and groundbreaking Emerge team on Chicago’s North side, the multidisciplinary team offers care coordination, individual therapy, family support, psychiatry, supported employment and education, and crisis intervention, designed specifically for the unique needs of older youth and young adults with mental health conditions, and delivered largely in the young adults’ home or community setting. The Community Support Individual team uses a team-oriented, strengths-based approach; this position is an opportunity to deliver creative therapeutic services in the context of a supportive team. LCSW or LCPC and experience with youth/young adults required.
