ESSENTIAL DUTIES AND RESPONSIBILITIES
Under the supervision of the Assistant Director and Director, the Case Manager’s responsibilities are to:
- Facilitate intake for individuals who are referred into interim housing
- Provide case management (linkage to income/benefits, supportive services, housing opportunities etc.) and follow-up services to a caseload of 15-25 individuals
- Perform a comprehensive needs assessment (screening and intake) for all case managed clients, and develop an individualized service plan that is based on client’s needs, barriers and goals
- Promote client independence by collaboratively establishing patient care goals, providing information to patients regarding their medical condition and medications, and supporting self-care skills.
- Create a supportive environment and ensure clients’ progress towards permanent supportive housing and psycho-social rehabilitation
- Utilize a client-centered approach that includes motivational interviewing, harm reduction, trauma informed care and housing first models
- Facilitate enrichment and life skills groups
- Provide crisis intervention when needed
- Work directly with TPC nursing staff on collaborative and whole person care
- Work directly with referring partners on oversight and acceptance of appropriate referrals
- Coordinate with primary care providers’ medical teams to ensure that clients’ medical needs are met.
- Coordinate medical transportation to support clients’ needs
- Maintain a cooperative relationship among care teams by communicating information, responding to requests, building rapport, participating in team problem-solving methods
- Maintain accurate documentation of client information (intake, referrals, progress notes and service delivery)
- Maintain accurate client records and statistical data (manually and via computerized database system)
- Participate in scheduled weekly clinical and case file meetings, monthly staff meetings, trainings and retreats
- Develop effective, trusting relationships with clients using a client centered approach that includes motivational interviewing, harm reduction, trauma informed care and housing first
- Utilizing harm reduction approach to provide health services support to individuals experiencing homelessness with acute and chronic medical conditions
- Collaborate with local Federally Qualified Health Centers (FQHCs) and other medical facilities to coordinate medical and mental health care for clients
- Communicate effectively and in a timely manner with management team, peers, collaborators, and clients
- Research available community resources and make referrals
- Coordinate information sharing among staff; consult as needed; coordinate and disseminate information to guests
- Facilitate bi-weekly caseload group and weekly House Meeting as needed
- Collaborate with other projects/agencies who share case management
QUALIFICATIONS
- BA or equivalent work experience in social services organization
- Minimum of two (2) year of experience in case management preferred
- CNA/MA background, experience coordinating medical care or experience in a healthcare environment preferred
- Demonstrated knowledge of and experience in dealing with issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)
- Skilled in non-violent crisis intervention
- Able to work in a high tolerance Harm Reduction and Trauma Informed Care model with members who have multiple barriers
- Detail oriented with strong time management, organizational, written, verbal, and computer skills
- Able to learn quickly and work effectively with a wide range of constituencies
- Strong interpersonal and team building skills
- Strong knowledge of Word Document, Excel and use of Emails systems (Outlook, 365, OneDrive) *Computer skill test may be requested during the interview. Please request reasonable accommodation in advance, if necessary.
- Knowledge of social services agencies and community resources
- Valid CA Driver’s license and an acceptable driving record
- Able to obtain and maintain CPR/1st Aid certification
