Schedule: Monday – Friday (8:30am – 5:00pm)

ESSENTIAL DUTIES AND RESPONSIBILITIES
Under the supervision of the Program Manager, the Case Manager is responsible:
- Provide crisis intervention, case management, and follow-up services to homeless individuals
- Provide information, referral services, and advocacy to support clients in achieving their goals
- Develop effective, trusting relationships with clients using a client-centered approach using best practices that include motivational interviewing, harm reduction, Trauma-Informed Care and housing first
- Perform a comprehensive needs assessment (screening and intake) for all case managed clients, and develop a plan that is based on client’s needs, issues and goals
- Maintain accurate documentation of client information (intake, referrals, progress notes, service delivery, etc.)
- Maintain accurate client records and statistical data (manually and via computerized database system)
- Educate community members (and potential clients) about Access Center services and the issues faced by the population served
- Participate in on-going self-evaluation, program evaluation, and development/modification
- Participate in all case file, staff meetings, training, and retreats
- Communicate effectively and promptly with the management team, peers, collaborators, and clients
- Report to work as scheduled
- Other duties as assigned
QUALIFICATIONS
- BA or equivalent 1-year work experience preferred
- Demonstrated knowledge of issues faced by population served (homelessness, mental illness, substance abuse, medical issues, domestic violence, etc.)
- Knowledge and experience working with the homeless population.
- Knowledge of social services agencies and community resources
- Able to work in a high tolerance Harm Reduction model with members who have multiple barriers
- Ability to self-motivate, learn quickly, work effectively with a wide range of constituencies and be flexible in a fast-paced environment
- Experience working in a community-based setting and as part of a team
- Committed to improving the community through empowerment and dignity
- Skilled in non-violent crisis intervention
- Computer literate; able to use computerized database system for client file management and knowledge of Microsoft Word, Outlook, and Excel
- Current, valid California Driver’s License with an acceptable driving record, reliable vehicle, and comfortable driving/transporting clients in agency van.
- Able to provide cleared TB test & pass a criminal background check
